There are several ways to start encounters in TotalMD: from the scheduler, the patient screen, the patient list and the encounter list.
From the Scheduler
From within the Scheduler, halo the patient in blue and right-click to get the drop down menu:
From the Patient Screen
Choose New Encounter from the left-hand menu:
From the Patient List
From the Patient List, choose the patient, right-click for the drop down menu:
From the Encounter List
To find the Encounter List, choose Lists and Encounter List:
You will then see the Encounter List of all Encounters, which can be sorted by any of the labels at the top of the table:
From this screen you can Modify Data in a variety of ways:
By choosing New Encounter, you will see the same pop up you get from the other three methods:
You will then choose the template you wish to use. These templates are created specifically for your practice. Then you will see a pop up to choose your provider for this encounter:
The first area to the right of the screen is the Encounter section. In this area you will add your Chief Complaint:
The second tab is for Diagnosis Codes:
You can see three sets of diagnosis codes:
- This Encounter’s Problems
- Problems from Previous Encounters
- Differential Diagnosis codes brought in through decision support rules and sections
You can choose to apply any/all of these to the current encounter. You will also see a Diagnosis List. This is the area you will use to choose diagnosis for this encounter. The Past Dx Codes will allow you to choose previously used diagnosis codes. Unused Dx Codes will allow you to add new diagnosis codes to this encounter.
The Procedures and Plan tab allows you to add CPT/HCPCS codes to the patient’s chart.
The Notes and Docs tab allows you to add notes that will not show up on the Clinical Summary:
The Orders tab allows for the Provider to order a variety of Labs and other paperwork:
Other Encounters allows you to see previous encounters with this patient:
To the left-hand side of the screen you will see Sections that are filled with the templates that will be created specifically for your use:
One type of template has areas that can be filled in free-text or with certain information from a drop down menu:
This template has some custom information to be chosen:
This form is strictly free-form text:
This is what your Clinical Summary looks like when you have finished the templates:
To print the Clinical Summary, you will go to the left-hand menu:
By Printing Clinical Summary, you will get this pop up:
From this screen you can choose what shows up on what you print for the patient. By unchecking the boxes, you can remove sections from the summary.
When you have finished with an Encounter, it can be forwarded to a co-worker for further action:
By doing the drop down, you can choose any other person in your system:
Once you choose another person, the encounter will then show up in the Encounter List as their Encounter:
Another function of this program is to assign a task for someone else:
All of the areas circled below should be filled in with drop down menus:
After saving the Task, it will place it in the Task List. You can find the Task List by going to the Lists drop down and choosing the Task List:

To see an individual Task List, go to the List drop down and choose Task List Window:
You will then see a list of only the Tasks assigned to the user who is currently signed into TotalMD:
