There are many keyboard shortcuts in TotalMD, but two of the most often used ones that will save you time in the Ledger if you are a keyboard person are covered here.
From within the Ledger, if you want to go to the bottom without scrolling if the patient has many transactions:
On your keyboard, you can hold down the Shift Key:
and then press F8:
which will take you to the end of their last transactions and get you ready to put in a new transaction:
Another good keyboard shortcut is that from anywhere in the Ledger if you hold down the Control button:
And then press the letter C on the keyboard:
It will take you to the area where you can choose another patient, without having to scroll up:
